Prohibits, under the Social Security Act, the disclosure of a person's social security number without the express, written permission of such person, unless required by law.
States that social security numbers issued under title II shall be issued solely for purposes of the old-age, survivors, and disability insurance program and no individual shall be required to include the number issued to him on any form prescribed for purposes of any Federal, State, or local law. Prescribes civil and criminal penalties for violations of the provisions of this Act.
