Provides that each Government agency that maintains any record concerning any individual which record may be retrieved by reference to or is indexed under the name of such individual and which contains any information obtained from any source other than such individual shall, with respect to such record: (1) notify such individual by mail at his last known address that the agency maintains or intends to maintain a record concerning such individual; (2) refrain from disclosing the record, or any information contained therein, to any other agency or to any person not employed by the agency maintaining such record, with specified exceptions; (3) maintain an accurate accounting of the name, addresses, and positions of all persons inspecting such record and the purposes for which such inspections were made and permit such individual to inspect such accounting; (4) permit such individual to inspect such record and have copies thereof made at his expense; (5) permit such individual to supplement any information contained in such record by the addition of any material containing information that such individual deems pertinent to the record; and (6) permit such individual to correct any information in such record by the deletion of any material containing information that such individual and the head of the agency determine to be erroneous.
